To foreigners wishing to have consultations with doctors (at our hospital):

当院受診の際の持ち物について
What to bring

 

1.日本に在住し、日本の公的保険にご加入の方
Patients who live in Japan and are covered with public health insurance:
・健康保険証
   Health Insurance Card
・在留資格証(在留カード)
Residence Card
・(ある場合)紹介状
If possible, a referral letter from another hospital or clinic.
※保険証を持ってこなかった場合は、自費診療(10割負担)、保険証がある場合は3割負担となりますので、保険証を忘れずにお持ちください。
   If you do not bring your Health Insurance Card, you will be asked to pay in full (100% of the total medical fee. If you bring your Health Insurance Card, you will only have to pay 30% of the total medical fee.

 

海外在住等で、日本の公的保険にご加入でない方、保険証をお持ちでない方(出張、観光など)
If the patient is based overseas and is not covered by Japanese public Insurance or don’t have the insurance card:
・パスポート
  passport
・(ある場合)紹介状
If possible, a referral letter from another hospital or clinic.

 

在留資格証(在留カード、パスポート等)と健康保険証確認のお願い
当院では診療申込みの際に、写真付き身分証明書(在留資格証、パスポートなど)を確認させていただきます。 在留資格が確認できない場合、緊急時を除き受診をお断りする場合がありますので、ご了承ください。
At the time of registration, patients are asked to show their Identification Cards which has their photos on it (Residence Card, Passport, etc.) If the patient cannot prove his/her residency, he/she may not receive the consultation at this hospital for the following reasons, except for serious emergency situations.

 

<理由>
Reason:
・写真付身分証明書での本人確認をし、患者さんの取違いによる重大な事故を防止するため
In order to avoid serious accidents such as misidentification of patients by confirming the patient with the Identification Card which bears the patients face.
・外国人患者受入れ医療機関認証取得のため、国籍や使用言語などの統計情報を報告する必要があるため
In order to gather statistical data, such as nationality and language used during the consultation, to obtain accreditation of ‘Japan Medical Service Accreditation for International Patients.’
・東京入国管理局より、在留資格証の確認を勧められているため
The hospitals are asked to confirm the Residency Card from the Tokyo Immigration Bureau

 

※なお、当院では、医療目的で来日される患者さんの受け入れは行なっておりません。

入院にかかるクレジットカード登録について
About credit card registration necessary for admission

 

●入院の場合:
For admissions:
入院受付時にクレジットカード登録用紙へのご記入とクレジットカードコピーをお願いしています。
At the time of admission, the patients are asked to fill in the credit card data in the registration form. The hospital will make a photocopy of the credit card.

英文の診断書、紹介状などの文書発行について
Obtaining Medical Certificates and Referrals in English

 

・英文の診断書や紹介状の発行は原則一部あたり¥11,000です。
Patients requiring Medical Certificates and Referrals must pay, in principle, 11,000 yen a copy.
・翻訳には数日掛かります。お急ぎの場合は、ご自身で翻訳会社等にご依頼ください。
It will take a few days for the translation. If you need them urgently, ask the translation company, etc., on your own.
・翻訳量が多い場合は追加料金を頂く場合があります。
If the translation is large in volume, an additional fee may be charged.

※ Consultations in red are for patients with appointments only. Please ask us for details.
※ Please note that the consultations in green differs in business hours, reception hours and doctors.
※ Prescriptions cannot be issued without consultations with the doctor.
※ As for non-consultation days, please look at the bulletin board in the hospital or check by phone.
※ Regular check-ups are done at the Internal Medicine Department (Reception time: 7:30 am to 10:00 am, 1:00 pm to 3:00 pm.) Some examinations may not be performed at the Department, so please check with the receptionist.
※ If you have any questions about the outpatient consultations and regular check-ups, please call 042-393-5511 (reception).

※症状によって受診いただけない場合がありますので、外来診療時間以外には事前に受診可能かどうか電話でご確認ください。
なお、当院では小児科・産婦人科の診察は行なっておりませんのであらかじめご了承ください。
※ Some patients may not be accepted for consultations after business hours (because the doctors on shift do not specialize in the field that can cope with the patient’s symptom.) We do not accept pediatrics patients and OB-GYN patients at our hospital.
※臨時で休診になったり、担当医が代わったりすることがあります。来院までに電話でお問合せください。その場合できるだけ、日本語を話せる方からのお電話をお願いいたします。
※ There are times when the hospital closes for certain reasons or when the doctors responsible for the day shifts to a locum doctor. Please call the hospital to make sure you can see your doctor. Also, please have somebody who can speak Japanese to make the phone call to make communication easier. 
※電話での対応ができないこともございます。外国語でのお問合せには、おかけになった電話番号に通訳を入れて折り返しいたしますので、電話番号が表示される設定にして電話をおかけください。
※ Sometimes, it is not possible to receive the phone call in English. When you cannot speak Japanese, we will have to call you back using the interpreting service, so please use the caller ID service (a mode in which your telephone number will be displayed while talking to the person on the other side of the phone) when you call the hospital.
